By filling out the required forms at home, you will be able to see the doctor quickly once you arrive at the office.  If you are unable to print out the above online forms, please call (817.488.8837) and ask our staff to FAX copies of the forms to you before your appointment.  Or, plan to come to the office at least 30-minutes early to fill out all forms, bringing with you all vital medical and insurance information, addresses and telephone numbers. Southlake Chiropractic is a Preferred Provider for more than 30 managed care plans and we will be happy to file insurance claims on your behalf.
